We work closely with family members as well as financial advisors, attorneys, health care providers, the Special Needs Trust trustee and others involved in the individual’s care.
Role Description
This is a hybrid contract role for a Care Manager located in to Greater St. Louis, with flexibility for work from home. The Care Manager will be responsible for guiding the course of care for older adults, assessing and planning for their needs, and maintaining their dignity, quality of life, and independence. The Care Manager will also provide support and direction to families, assisting with care transitions and providing education and resources.

Case Management and Elder Care expertise

Experience in training and social services

Understanding of medicine and healthcare systems

Excellent communication and interpersonal skills

Compassionate and empathetic approach to care

Ability to work independently and remotely

Master's degree in Social Work, Nursing, Counseling, OT, or related field

Certified Care Manager (CCM) Licensed Certified Social Worker (LCSW) or similar credentials is a plus

Role Description
This is a contract hybrid role for a Social Worker at Aging Gracefully. The Social Worker will be responsible for assessing and planning the needs of older adults, providing support to individuals and families, and maintaining their dignity and independence. The day-to-day tasks include conducting assessments, developing care plans, coordinating services, providing counseling and emotional support, and collaborating with healthcare professionals and other organizations. This is a hybrid role, with the expectation of some remote work while being primarily located in Greater St. Louis.
